The Block Center: Gathering the Information We Need to Give You the Best Care
At the Block Center, we believe the best care begins with a doctor patient relationship built upon open and honest communication. This process begins with a comprehensive 10-part patient questionnaire. By obtaining a detailed case history from you, we gain the tools we need to tailor a program that addresses your biological, physical, biochemical, and emotional needs.
When you contact us, we will mail you a copy of the questionnaire. Should you find any part of it difficult, or if you would feel more comfortable if one of our Patient Advocates were to assist you in filling it out, please do not hesitate to let us know. And, although we believe we can provide you with far better care if we are armed with the most complete history as early as possible in our treatment partnership, we are always sympathetic to your needs: If you do not want to fill out the questionnaire, just inform one of our staff members that you prefer not to do so, and we will proceed without it.
Please be assured that the absolute confidentiality of the doctor-patient relationship applies to our entire staff and to all materials in your case history file, including the questionnaire material.
